The Role of the R.N. Team Leader is to promote and restore patients’ health by maintaining the day to-day operation of the patient care area, giving guidance and direction to the patient care teams to plan, direct and evaluate the total nursing care and functions, in accordance with established philosophy and reflective of Sun Life’s Mission and Vision. The RN Team Leader coordinates care for patients on the provider’s daily schedule and proactively manages and coordinates care for patients not on the schedule.

  • Relates effectively and maintains satisfactory relations and rapport within other departments, providers, employees, and visitors
  • Provide an ongoing evaluation of personnel performance, staff education, orientation, training, monitoring and in-services and make assignments to the skill level of employees. Supervise the patient care team that report to you. Identify employee problems and guide them to their solution. Report to Director of Nursing any issues in a timely manner
  • Interact with patients and families in a tactful manner and resolve patient grievances/complaints
  • Refer any unresolved patient grievances to Director of Nursing to be addressed. Document all interactions
  • Review current or new plan of care to include Healthy Lifestyle Goal Settings with patient and perform patient education activities as directed by provider
  • Facilitates the completion of pre-visit patient information
  • Review preventative care needs of the patient and alert the provider to provide orders for such. Examples are male or female preventative exams, immunizations and other recommended age related screenings
  • Participate in patient education for example family planning, diabetes, well child and medication management
  • Provide the best possible nursing care by planning, organizing, and directing the nursing functions of patients
  • Identify patient service requirements by establishing rapport with the patient and others while being aware of age specific needs protocols. Exhibits knowledge of growth, development and age appropriate communication skills. Ability to assess, provide and interpret age specific data. Ability to involve family or significant other in decision-making related to plan of care
  • Facilitate active recall on those patients who need chronic care management services and preventative care. Examples of chronic care services include diabetes care, pre-hospitalization follow up and hypertension. Examples of preventative care services include well child visits, immunizations and mammogram
  • Facilitates the set up of rooms, placing patients in rooms, taking vital signs, height, weight, pulse ox, oxygen levels, etc. and documentation in the chart, preparing patients for examination, performing EKG’s on patients, and assistance with patient care such as applying braces, ace wraps and wound dressings
  • Triage patients and record all appropriate documentation in patient medical record, in a timely and professional manner
  • Monitor medical condition of patients and report changes to provider if deemed necessary and if required perform IV initiation and monitor intravenous solutions skillfully and correctly
  • Facilitates timely responses to patient telephone calls and messages
  • Act as a liaison with the providers, other doctor’s offices, and the hospital including receiving reports on patients and delivering them to the appropriate provider
  • Ensures referrals if required (those not processed by referral department) and set up and arrange for patient to receive home health visits. Set up appointments for patient for diagnostic procedures and/or with a specialist
  • Clarify doctors recommendations for patient medications and directions for use; or changes made at time of patient appointment. Call in prescription refills as directed by the provider. Support patients and providers in the medication refill process. Notify patients when prescription orders are ready, faxed or submitted to pharmacy. Complete prior authorizations for medications
  • Notify patient of test results when instructed to do so by provider by phone or letter
  • Assure quality of care by maintaining clinic standards of care while adhering to requirements of other governing agencies including reporting communicable disease to County Health Department as needed
  • Work with staff to become and maintain recognition with the Nation Center for Quality Assurance as a Patient Centered Medical Home
  • Protect patients and employees by complying with infection control policies and protocols, following medication administration, storage procedures and controlled substance regulations. Maintain Universal precautions and follow all applicable SLFHC procedures regarding infection control and reducing healthcare acquired infections (HAIs). This includes but is not limited to, good hand-washing and gloving practices
  • Following medication administration, storage procedures and controlled substance regulations and monitoring staff for compliance on these issues
  • Maintain documentation of patient care services by completing patient and department records in accordance with established policies and procedures. Administer and document medications, immunizations correctly
  • Participates with the process to ensures that out appropriate immunization paperwork is completed for recording purposes and document in ASIIS
  • Document verbally and in writing procedures, medication, delivery of care, and pertinent information regarding care rendered to the patient. Routine and timely documentation of all core process work activities including, but not limited to, evidence-based chart reviews, patient calls & correspondence
  • Ensure operation of medical equipment by completing preventive maintenance requirements; following manufacturer’s instructions; reporting malfunctions, requesting repairs, maintaining equipment inventories, and evaluating new equipment and techniques
  • Maintain Clinical supplies inventory by anticipating needed supplies. Ensures that rooms are stocked with adequate medical supplies, wash and dry instruments used, and prepare for sterilization as required. Ensures or performs sterilization of instruments and surgical packs in a timely manner
  • Perform venipuncture, specimen collection and laboratory CLIA waived testing for which he/she has received training
  • Maintain Clinic standards of care by adhering to the Arizona State Nurse Practice Act and those of other governing agencies. Adhere to expectations of required/mandatory in-services, certifications, and licensure
  • Assist the Director of Nursing of employee health screenings and compliance with required Education
  • Monitors and ensures that all required Audits are completed in a timely manner, including chronic/preventative care recall for patients
  • Conduct monthly Safety audits and participate in Wellness monitoring activities
  • Assist personnel with computerized timekeeping system and ensure all staff have hours correct and approved in a timely manner
  • Ability to handle protected health information (PHI) in a manner consistent with the health insurance portability and accountability act of 1996 (HIPAA)
  • Act as a backup to other patient care team members as needed
  • Works collaboratively with the Director of Nursing to ensure that processes and procedures developed are "institutionalized" in the delivery of patient care


  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; and establishing personal networks.
  • Participate in in-service/education regarding Quality Improvement.
  • Participate in planning changes and improvements.
  • Perform all other related duties as assigned or requested.

  • Registered nurse (R.N.) with valid state license
  • ADN, AAS degree required
  • One year of nursing experience as an RN or LPN required
  • Cardiopulmonary Resuscitation (CPR) Health Care Provider Level current
