ESSENTIAL FUNTIONS

STATEMENT OF PURPOSE:

Community Outreach is the process of communication, promotion and education to communities, stakeholders and individuals regarding the organization and its services, the availability of medical assistance programs, and the organization’s resources for enrollment and/or assistance with the application process. The Outreach & Enrollment Specialist is responsible to assist with outreach and promotion of the organization’s programs and services. Job responsibility includes application assistance interviews with clients, screening for financial qualification for state or federally funded Medical Assistance, federally facilitated insurance marketplace or Discount Programs, as well as participation in community relations activities.

ESSENTIAL FUNCTIONS:

  • Maintain established policies and procedures, objectives, quality assurance and safety standards within the department
  • Educate families about the availability of publicly financed health care programs and insurance options through the federally facilitated marketplace, and organization’s sliding fee discount programs, and provide application assistance to by performing eligibility screening including compiling necessary documentation
  • Educate families about the availability of programs offering free or low cost prescription medications, and provide application assistance by performing eligibility screening for the programs including compiling necessary documentation
  • Educate families about the availability of nutrition assistance programs, and provide application assistance by performing eligibility screening for the program including compiling necessary documentation
  • Update files daily to reflect the status of the application, and coordinate with agencies to expedite the eligibility determination of applicants
  • Manage appointment schedule to maximize client intake appointments for the department. Reschedule clients when necessary
  • Participate in and assist with internal audits of discount programs. Participate in external audits of program applications
  • Assist with implementation of Community Outreach strategic plan, to facilitate growth of patient base, profit growth and expansion of the organization’s programs and services
  • Assist with education of staff members, including providers, on non-medical and safety net programs available to SLFHC patients
  • Provide referral to SLFHC patients to non-medical and safety net programs. Track referrals in SLFHC electronic health record
  • Assist with Stakeholder Development. Represent organization to, develop and maintain relationships with community based organizations / referral sources and other entities serving mutual clients.
  • As directed, participate in external committees and meetings within SLFHC service area(s). Serve as an organizational representative for relevant community events and activities
  • Assist in coordination and/or participation in volunteer and community based initiatives
  • Maintain thorough knowledge of Health-e-Arizona Plus, and on qualification requirements for Medicaid, federally facilitated insurance marketplace and SLFHC Discount Programs
  • Maintain annual certification as Certified Application Counselor (CAC) as required by the U.S. Department of Health and Human Services, and required licensure through the Arizona Department of Insurance
  • Enhance professional growth and development through participation in educational programs, review of current literature, in-service meetings and workshops
  • Help to operate department within established budget

ADDITIONAL RESPONSIBILITIES:

  • Participate in in-service/education regarding Quality Improvement
  • Perform all other related duties as assigned or requested

  • Developed skills in the Microsoft Office programs: Word, Publisher, PowerPoint, Excel and Outlook

REQUIREMENTS

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Understanding of issues of concern to low-income and minority communities
  • Appreciation of the cultural sensitivities of people from a wide range of backgrounds
  • Ability to increase the understanding of the importance of prevention and medical treatment
  • Ability to communicate effectively with patients and employees of the eligibility agencies
  • Ability to communicate effectively with monolingual Spanish speaking patients
  • Ability to perform basic calculations using whole numbers, fractions and decimals
  • Ability to work under stressful situations
  • Computer literate

EDUCATION AND EXPERIENCE:

  • High school diploma or equivalency
  • Associates degree in related field preferred
  • Minimum three years experience working in a responsible position where the primary emphasis included contact with the public
  • Strong familiarity with human service agencies servicing low-income and minority populations
